Enrollment paperwork isn't just admin — it's the record you'll reach for in the moments that matter. The pick-up list is your authority to say "I'm sorry, I can't release them to you" at the door. The allergy section is what a substitute or family member checks before serving lunch. The permission initials are what stand between you and an awkward conversation about a photo in the daily update.
A few habits that make this form work harder:
- Have each permission initialed separately rather than covered by one signature at the bottom. Parents notice what they're agreeing to, and you have a clearer record if a policy is ever questioned.
- Refresh it yearly and whenever anything changes — phone numbers and authorized pick-ups drift more than you'd expect. A quick re-sign at your program's anniversary keeps the file honest.
- Keep the custody notes section current. If there's a court order, attach a copy — you can only enforce arrangements you have on file.
- Check your state's licensing list. Most states also require an immunization record on file, and some add their own emergency-consent or health-history forms. This template covers the universal core; your licensor's checklist has the local extras.
