How to use it

The form you hope to never need — kept where you'd need it.

Your enrollment form probably collects this information already — so why a standalone page? Because in an actual emergency, nobody flips through a two-page enrollment packet. One sheet per child, in a grab-and-go binder by the door (the same one that leaves with you on walks and field trips), is the difference between calling the right person in ten seconds and searching a filing cabinet with shaking hands.

  • Keep the call order honest. Ask parents to list whoever actually answers their phone first — the parent in back-to-back meetings shouldn't be line one if grandma two blocks away picks up instantly.
  • Re-collect every enrollment year, and any time a family mentions a new number in passing. Phone numbers rot faster than any other data you keep — an annual re-sign is the only reliable defense.
  • Fill the five-second allergy line thoughtfully. It's not the full medical record — it's what a first responder or covering adult needs to know immediately: "peanut anaphylaxis — EpiPen in red cabinet."
  • Take it with you. Walks, outings, evacuations — the binder of emergency sheets travels. Many states require exactly this; all common sense does.
