Incident reports

Incident reports document injuries and other events for a child's file and for licensing. Each report is tied to a child but is retained even if that child is later removed — the child's name and age at the time are saved onto the report, so your records stay complete.

Filing a report
  1. Open a child (Children → the child), then 🩹 Incident reports, or use the 🩹 icon on the Children list.
  2. Click File a report, set the date, time, and location, and fill in what happened.
  3. Wylari records the program name, the child & age, and who filed it (your name) automatically.
  4. Click Save report. Use Print for a clean copy with signature lines for staff and parent.
Standard fields

Every report includes cause, injury locations and types, medical treatment and facility, witnessing staff, equipment involved, other children involved, and prevention steps. Blank fields are simply left off the printed report.

Custom fields

Under Settings → Incidents you can add your own fields (for example "Parent notified?" or a licensing-specific question) as short text or a paragraph. They appear on every report form.

The recent-report badge

The Children list and the dashboard flag any child with reports filed in a recent window — 30 days by default. Change the window under Settings → Incidents. It's a quick way to spot a child who has had several incidents in a short time.
